About the Role

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at HCA Virginia Healthcare at Home in Fredericksburg. As a PRN Home Health LPN, you will have the opportunity to provide high-quality patient care in a dynamic and supportive environment.

Job Summary

As a PRN Home Health LPN, you will be responsible for administering quality, skilled nursing care to patients in their homes. You will work in accordance with physician orders and the Plan of Care under the direction and supervision of a Registered Nurse and/or Director, Professional Services. Your primary focus will be on providing patient-centered care, identifying physical, psychosocial, and environmental needs, and collaborating with other healthcare professionals to ensure the provision of quality patient care.

Responsibilities
  • Provide patient care and identify physical, psychosocial, and environmental needs of patients.
  • Verify Plan of Care prior to each visit and provide care according to physician orders, assessment data, established standards, and guidelines.
  • Perform ongoing assessments during each visit and document data in patient medical records.
  • Communicate significant findings, problems, or changes in patient condition to supervising RN and/or Director of Professional Services and physician.
  • Assess patient/caregiver's knowledge of disease process, treatment regimen, and/or medication administration and provide basic and specialized teaching as ordered in the Plan of Care.
Requirements
  • Basic Cardiac Life Support certification within 30 days of employment start date.
  • Licensed Practical/Vocational Nurse (LPN/LVN) licensure.
  • Valid driver's license.
  • Vocational school graduate.
