BAYADA Home Health Care is seeking a skilled Home Health Physical Therapist to join our team in Mechanicsville, Virginia.
About the JobWe are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our adult and geriatric clients in their homes. As a Home Health Physical Therapist, you will be an integral member of a multidisciplinary healthcare team, working closely with nurses and other healthcare professionals to develop and implement individualized care plans.
Responsibilities- Make home visits to clients in designated geographic territories to assess their physical needs and develop personalized care plans.
- Perform diagnostic tests and measurements to evaluate clients' mobility, strength, and balance.
- Develop and implement appropriate interventions, including manual therapeutic exercises, gait training, and balance activities.
- Educate clients and their families on rehabilitative care and activities to promote independent living.
- Accurately document observations, interventions, and evaluations in a state-of-the-art electronic medical record system.
- A current VA Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
