Complete Staffing Solutions is seeking a highly motivated and experienced Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to clients in need of homeless, mental health, and substance abuse services and resources.
Key Responsibilities:
- Provide case management services to clients, including assessment, goal setting, and service planning.
- Develop and maintain relationships with community resources and service providers to ensure access to necessary services.
- Conduct home visits and provide in-home support to clients as needed.
- Collaborate with other staff members to ensure comprehensive support to clients.
- Maintain accurate and up-to-date records of client interactions and services provided.
Requirements:
- Minimum of one year relevant experience with homeless, mental health, and substance abuse services and resources.
- Valid driver's license.
- Proficient computer and technology skills, including Microsoft Word and Excel, and other web-based applications.
- Flexible, reliable, motivated, and possess a positive attitude.
- Excellent time management skills and ability to meet deadlines.
- Organized, able to multi-task, and follow-through with tasks to completion.
- Work well independently and as part of a team environment.
Benefits:
- $23/hour pay rate.
