Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our team at the City of Somerville. As a Case Manager, you will play a critical role in providing comprehensive and customized case management services to residents in need of support for their mental, social, and physical well-being.

Key Responsibilities
  • Conduct assessments and intakes of clients to identify their needs and develop personalized plans to address their concerns.
  • Develop relationships and collaborate with community-based organizations, government agencies, and other City departments to identify and provide access to resources and services for clients.
  • Provide referrals to community agencies for needs such as mental health, housing, homelessness, and substance use.
  • Maintain accurate and up-to-date client databases to track efforts and outcomes, and produce required reports.
  • Stay informed about social service resources offered by the City, State, and non-profit agencies, and develop strong partnerships with community organizations and service providers.
  • Conduct workshops and training sessions for internal City staff and external partner agencies on case management and oversight best practices.
  • Assist clients with navigating forms and systems to access programs and services, and facilitate seamless referrals to internal and external programs.
  • Attend local coalition meetings and develop social media content, web content, and publicity materials in collaboration with the City's Communications Department.
Requirements
  • Bachelor's Degree and 3-5 years of experience in case management services, or any equivalent combination of education, training, and experience.
  • Licensed Social Workers (LSW) are strongly encouraged to apply.
  • CPR/First Aid Certification within three months of hire and ongoing maintenance.
  • Ability to travel independently to multiple locations within the City to complete work assignments.
