Nurse Case Manager

Mass General Brigham is seeking a skilled Registered Nurse to join our team as a Case Manager in Somerville, Massachusetts.
About the Role:This is an exciting opportunity to work in a multidisciplinary team that plays a central role in managing capacity on behalf of the eleven hospital Mass General Brigham system.
As a Case Manager, you will work closely with teammates, colleagues, and stakeholders to enable safe, efficient, and timely patient care by serving as the unified hub for system capacity management.
Key Responsibilities:- Monitor system-wide capacity and develop strong knowledge of hospital and non-hospital assets throughout Mass General Brigham, as well as capabilities at non-Mass General Brigham facilities.
- Strive to always match patient need with site of care to optimize capacity and access across the Mass General Brigham system.
- Manage and coordinate Repatriations across Mass General Brigham, including proactively identifying candidates, preparing and sending hospital teams, tracking patients during their hospital stay, and coordinating with unit-based Case Managers to initiate Repatriation.
- Escalate barriers encountered during Repatriation and receive support from Mass General Brigham Patient Transfer & Access Center teammates and leadership.
- Document Repatriation activity in the appropriate databases and maintain data and create reports summarizing Repatriation volume, barriers, and impact.
- Provide guiding recommendations to Mass General Brigham PTAC leadership regarding how to improve Repatriation.
- Participate in quarterly meetings with stakeholders at hospitals and hospital systems that are key partners for Repatriation.
- Build and maintain strong relationships with colleagues at hospitals that are partner destinations for Repatriation.
- Help the Mass General Brigham PTAC develop new relationships with hospitals and systems that have historically not participated in Repatriation with Mass General Brigham.
- Teach and guide hospital-based Case Managers on Repatriation.
- Identify and manage scenarios in which a patient may benefit from alternative levels of care or alternate placement, including out-of-network transfer requests, out-of-state and transfer requests with anticipated difficult disposition, and home-based services.
- Work closely with colleagues in the Specialty Desks within the Mass General Brigham PTAC to help address barriers to discharge that are escalated from the Mass General Brigham hospitals or other assets.
- Provide hospital-based Case Management leaders with summary situational awareness about system-wide hospital capacity, post-acute availability, transportation, and other domains critical to outflow within the scope of Case Management.
- Participate in Mass General Brigham huddles representing Case Management.
This role requires a strong understanding of healthcare systems, capacity management, and patient flow. The ideal candidate will have excellent communication and collaboration skills, with the ability to work effectively in a fast-paced environment.
