Nurse Case Manager

Mass General Brigham is seeking a skilled Registered Nurse Case Manager to join our team in Somerville, Massachusetts.
Job Summary:The Registered Nurse Case Manager will be responsible for coordinating and managing patient care to ensure optimal outcomes. This includes assessing patient needs, developing and implementing care plans, and collaborating with healthcare teams to ensure seamless transitions of care.
Key Responsibilities:- Coordinate and implement patient care plans to achieve desired clinical and financial outcomes.
- Assess patient needs and develop individualized care plans to address their unique requirements.
- Collaborate with healthcare teams, including physicians, nurses, and other healthcare professionals, to ensure effective communication and coordination of care.
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
- Participate in ongoing evaluation of practice patterns and systems to identify opportunities for improvement.
- Contribute to the development and implementation of practice guidelines and protocols to enhance patient care.
- Graduate of an approved school of nursing with current registration in Massachusetts.
- Bachelor of Science Degree in Nursing required for newly licensed nurses and external candidates.
- 1+ years of acute care experience required.
- 1 year of case management experience required.
- Previous experience in a hospital or healthcare setting.
- Bilingual (English/Spanish) preferred.
