Job Description:

VitalCheck Wellness is seeking a Corporate Wellness Consultant LPN to coordinate our Diabetes and Chronic Disease Management Program. The ideal candidate will be passionate about improving patients' lives and delivering high-level customer service.

Responsibilities:
  • Coordinate between patient, clinical, and operations partners to set up appointments, follow up, and gather feedback.
  • Ensure high adherence rates to the program in accordance with our guidelines.
  • Provide education on diabetes management and exercise advice to help clients establish healthy eating habits and lifestyles.
  • Support clients in diabetes management and other aspects of well-being and lifestyle.
  • Establish and maintain rapport and good relationships with patients.
  • Maintain clients' documentation.
Requirements:
  • Bachelor's degree in nursing, health administration, or a related field.
  • Active license of Licensed Practical Nurse (LPN) in NY or NJ (preferred).
  • Three to five years of experience in nursing, health administration, or a related field.
  • Experience working with diabetes patients. Certified Diabetes Educator strongly preferred.
  • Experience with vaccine and phlebotomy strongly preferred.
  • Proficient with computers, Microsoft Office, EMR systems.
  • Customer service-oriented and have great bedside manner.
  • Strong communication skills; both written and verbal.
  • Ability to work independently with little supervision.
  • Bilingual with Haitian Creole or Spanish speaking strongly preferred.
  • Must be able to reliably commute throughout NYC and/or NJ.
