Job Description:VitalCheck Wellness is seeking a Corporate Wellness Consultant LPN to coordinate our Diabetes and Chronic Disease Management Program. The ideal candidate will be passionate about improving patients' lives and delivering high-level customer service.
Responsibilities:- Coordinate between patient, clinical, and operations partners to set up appointments, follow up, and gather feedback.
- Ensure high adherence rates to the program in accordance with our guidelines.
- Provide education on diabetes management and exercise advice to help clients establish healthy eating habits and lifestyles.
- Support clients in diabetes management and other aspects of well-being and lifestyle.
- Establish and maintain rapport and good relationships with patients.
- Maintain clients' documentation.
- Bachelor's degree in nursing, health administration, or a related field.
- Active license of Licensed Practical Nurse (LPN) in NY or NJ (preferred).
- Three to five years of experience in nursing, health administration, or a related field.
- Experience working with diabetes patients. Certified Diabetes Educator strongly preferred.
- Experience with vaccine and phlebotomy strongly preferred.
- Proficient with computers, Microsoft Office, EMR systems.
- Customer service-oriented and have great bedside manner.
- Strong communication skills; both written and verbal.
- Ability to work independently with little supervision.
- Bilingual with Haitian Creole or Spanish speaking strongly preferred.
- Must be able to reliably commute throughout NYC and/or NJ.
