Job Summary

We are seeking a dedicated RN Case Manager to join our dynamic team. As a RN Care Coordinator, you will spend quality one-on-one time with patients and their families to support everyday needs.

Responsibilities
  • Provide initial and ongoing assessments of patient and family needs.
  • Create and initiate care plans through assessing the history of current and previous illnesses.
  • Record important health information including temperature, pulse, respiration, and blood pressure.
  • Administer medications and treatments as prescribed by the physician.
  • Provide health care instructions to the patient as appropriate.
  • Record symptom management changes and outcomes.
  • Complete, maintain, and submit accurate and relevant clinical notes regarding the patient's condition and care given.
Qualifications
  • An enthusiasm for providing the best possible care to patients and their families.
  • An RN license in good standing.
  • 1 year of experience in hospice or home health preferred.
  • A valid driver's license.
  • Current CPR certification.
  • Excellent observation, verbal, and written communication skills.
