Job Summary

We are seeking a Day Program Director to oversee the management of personnel and activities at our PACE Center. The ideal candidate will have a strong background in supervising staff, coordinating services, and leading interdisciplinary teams.

Responsibilities
  • Manage personnel and activities within the PACE Center, including hiring, training, and scheduling.
  • Supervise staff, including evaluation of performance, counseling, and discipline.
  • Coordinate services to frail elderly participants, including emergency arrangements, transportation schedules, and care coordination.
  • Lead and coordinate the PACE Interdisciplinary Team (IDT), including RN, LPN, Licensed Social Worker, RD, Activity Coordinator, and other healthcare professionals.
  • Collaborate with the Center Director and Quality Department to ensure adherence to state regulations and staffing ratios.
  • Engage with PACE Administration to further program development and participate in planning, development, and implementation of program goals and objectives.
  • Maintain departmental policies and procedures.
  • Participate in the development of departmental budget under the direction of the Center Director.
  • Oversee the quality of care and safety of PACE participants in the center and community.
  • Directly supervise Participant Care Associates and other center support staff.
  • Serve as a liaison between disciplines, administration, and direct staff.
  • Investigate, monitor, and track PACE participant grievances and incidents to ensure prompt resolution.
Requirements
  • Bachelor's degree required.
  • Minimum of two years supervisory experience required.
  • Minimum of one year experience working with a frail or elderly population.
  • Interest and ability to work in an interdisciplinary team environment.
  • Group leadership skills.
  • Ability to thrive in a fast-paced environment and handle difficult interpersonal situations.
Compensation

$73,000 - $83,000 per year, depending on experience and location.
