Pathway of Hope Case Manager

Job Summary:
The Salvation Army is seeking a highly motivated and compassionate Pathway of Hope Case Manager to join our team. As a Case Manager, you will provide strengths-based intensive case management services to families who desire to break the cycle of intergenerational poverty and move towards self-sufficiency.
Key Responsibilities:
- Conduct comprehensive assessments and develop individualized action plans for clients
- Provide ongoing support and expertise through case management and referrals to community resources
- Collaborate with The Salvation Army Corps and community partners to provide a holistic and wrap-around approach
- Track client data and maintain accurate records using WellSky Community Services
- Participate in trainings and meetings to stay up-to-date on best practices and program requirements
Requirements:
- Bachelor's degree in Social Work or related field, or minimum of 2-3 years of case management experience
- Valid California driver's license and ability to pass a criminal background check
- Minimum one year of experience working with low-income and/or homeless families
- Ability to work with diverse populations in a trauma-informed and sensitive manner
Working Conditions:
This is a full-time, non-exempt position that requires frequent travel to community locations. Business casual attire is expected during office and client visits. Attendance in trainings and meetings is required.
