Job Summary:

The Salvation Army is seeking a highly motivated and compassionate Pathway of Hope Case Manager to join our team. As a Case Manager, you will play a vital role in providing strengths-based intensive case management services to families who desire to break the cycle of intergenerational poverty and move towards self-sufficiency.

Key Responsibilities:

  • Conduct comprehensive assessments of clients' needs, utilizing a strengths-based perspective and motivational interviewing techniques.
  • Develop and implement individualized action plans, setting goals and objectives for clients to achieve.
  • Provide ongoing support and expertise to clients, including referrals to community resources and services.
  • Collaborate with The Salvation Army Corps and community partners to ensure a holistic and wrap-around approach to meeting clients' needs.
  • Maintain accurate and thorough client data, utilizing WellSky Community Services.
  • Respect and adhere to client privacy guidelines, including HIPAA and relevant POH guidelines.

Requirements:

  • Bachelor's degree in Social Work or related field, or minimum of 2-3 years of case management experience.
  • Valid California driver's license.
  • Minimum one year of experience working with low-income and/or homeless families.
  • Knowledge of the unique issues of homelessness and ability to work with diverse populations in a trauma-informed, sensitive, and non-judgmental manner.
  • Basic understanding of mental health disorders and proficiency in Microsoft Windows and use of Word, Excel, and PowerPoint.

Working Conditions:

The Case Manager will work in a fast-paced environment, often in the community, with some desk work. Business casual attire is required during office and client visits. May need to wear business attire for certain functions. Attendance in trainings and meetings is required.
