Job Title: Pathway of Hope Case Manager

Join The Salvation Army team as a Pathway of Hope Case Manager and make a meaningful difference in the lives of families in need. As a key member of our team, you will provide intensive case management services to families who desire to break the cycle of intergenerational poverty and achieve self-sufficiency.

Job Summary:

The Pathway of Hope Case Manager will work closely with clients to assess their housing, health, financial, and psychological needs, and develop individualized action plans to address these needs. You will provide ongoing support and expertise through comprehensive assessment, planning, implementation, and evaluation of clients' needs, utilizing a strengths-based perspective and motivational interviewing techniques.

Key Responsibilities:
  1. Conduct intake and assessments to determine client eligibility for Pathway of Hope services and financial assistance.
  2. Develop individualized action plans and set goals for each client, providing ongoing case management and assessments at 30, 60, 90, and 12 months, with additional follow-up after completion of the program.
  3. Provide referrals to clients on housing, health benefits, financial support, and community services.
  4. Record and maintain accurate client data using WellSky Community Services.
  5. Respect and adhere to client privacy guidelines, including HIPAA and relevant Pathway of Hope guidelines.
  6. Support clients' spiritual growth by connecting them with pastoral care representatives.
  7. Network with outside agencies and build relationships with community partners.
  8. Maintain a current file of community resources for staff use.
  9. Attend monthly case management meetings and participate in meetings with the Divisional Pathway of Hope Coordinator.
  10. Compile monthly statistical reports as required by the Division.
Requirements:
  1. Bachelor's degree in Social Work or related field, or minimum of 2-3 years of case management experience.
  2. Valid California driver's license.
  3. Minimum one year of experience working with low-income and/or homeless families.
  4. Ability to pass a criminal background check.
Preferred Qualifications:
  1. Bilingual English/Spanish.
  2. Knowledge of the unique issues of homelessness and mental health disorders.
  3. Ability to work with diverse populations in a trauma-informed, sensitive, and non-judgmental manner.
Working Conditions:

This is a full-time, non-exempt position that requires a high level of motivation and self-starter skills. Most work will be done in the community, with some desk work. Business casual attire is required during office and client visits. Attendance in trainings and meetings is mandatory.
