Reports to the Manager or designated representative. Oversees resources through collaborative interdisciplinary efforts to achieve optimal outcomes for oncology patients. Integrates research data, community healthcare providers, and Beacon Health System resources into a cohesive model of access and care that benefits patients, involved physicians, and family members. Acts as an ambassador for the hospital and Beacon Medical Group from the patient's initial contact through follow-up post-treatment for coordinated care. Provides education regarding available services for oncology patients across various specialty departments within the Cancer Center. Develops comprehensive survivorship care plans for all eligible oncology patients and offers teaching and resources for effective survivorship care.

Coordinate oncology patient care throughout the continuum of their treatment by:
  • Assisting adult oncology patients in navigating the healthcare system.
  • Conducting initial discussions with newly diagnosed patients (and families) to identify any needs or barriers that may affect their engagement in care.
  • Networking with physicians and other healthcare providers to ensure effective execution of patients' care plans and the achievement of desired outcomes.
  • Acting as a liaison between families and the interdisciplinary team by educating and assisting with understanding the care plan for patients, families, and other healthcare team members.
  • Providing support to patients with concerns related to insurance, transportation, childcare, financial resources, language barriers, and education; managing patient concerns to allow them to focus on recovery.
  • Initiating referrals within Beacon Medical Group and other community organizations to connect patients with necessary resources to eliminate barriers to care, such as pain management, financial counseling, transportation access, language assistance, childcare, complementary services, nutritional referrals, etc., with the aim of helping patients manage concerns to focus on recovery.
  • Collaborating with community organizations to coordinate referrals as necessary.
  • Utilizing interdisciplinary care conferences to plan interventions for patients.
  • Encouraging patient adherence to medical instructions and referrals by educating patients and families and supporting physicians' treatment recommendations as needed.
  • Reporting variance trends to the manager/director or the appropriate Physician reviewer for evaluation and action.
  • Coordinating the collection and reporting of patient outcome information, including follow-up with patients after procedures.
  • Developing and discussing a survivorship plan that outlines treatment received, potential long-term effects of treatment, and follow-up guidelines.
  • Conducting health screenings and providing educational offerings within the community.
  • Maintaining an active role in the Oncology Care Committee, reporting all activities to the group and the American College of Surgeons as required.
Participate in continuous quality improvement by:
  • Compiling and analyzing data necessary for generating reports; ensuring data accurately reflects utilization trends and patterns.
  • Preparing summaries and reports for review by the Oncology Care Committee.
Coordinate the activities of multidisciplinary oncology clinics by:
  • Managing patients within each individual oncology clinic and/or BMG clinic.
  • Providing direct nursing care for multidisciplinary clinics.
  • Coordinating scheduling of necessary procedures and follow-up care.
Perform additional functions to maintain personal competence and contribute to the overall effectiveness of the department by:
  • Completing other job-related assignments and special projects as directed.
Coordinate Survivorship activities by:
  • Utilizing the analytic cases list from the Cancer Registry to identify patients eligible for a treatment summary and survivorship care plan.
  • Developing survivorship care plans for all eligible patients.
  • Delivering and discussing survivorship care plans with eligible patients and their families.

  • Direct patient care providers are required to maintain current BCLS (CPR) and other certifications as required by position/department.
  • Consistently utilize appropriate universal precautions, protective equipment, and ergonomic techniques to protect patients and self.
  • Adhere to regulatory agency requirements, survey processes, and compliance.
  • Comply with established organization and department policies.
  • Be available to work overtime in addition to working additional or other shifts and schedules when required.

Education and Experience
  • The knowledge, skills, and abilities as indicated below are typically acquired through the successful completion of an accredited nursing program. A current license to practice as an RN in Indiana and an Oncology Certified Nursing (OCN) certification are required. If not certified, certification is required within two years of hire. A minimum of five years of oncology nursing experience is preferred. Prior experience in a leadership role (which utilized problem-solving and conflict management processes) in a healthcare setting is required.

  • Requires thorough knowledge of community agencies, services, programs, and resources available to assist patients and their families.
  • Requires knowledge of counseling techniques and the ability to identify and utilize appropriate resources.
  • Demonstrates the communication skills (including verbal, written, and listening) necessary to effectively counsel patients and their families.
  • Demonstrates the interpersonal skills necessary to promote and maintain cooperative, courteous, and sincere relationships with patients, family members, physicians, staff, and the public.
  • Demonstrates sensitivity and the ability to use tact when working with patients and their families.
  • Requires time management and problem-solving skills and the ability to independently prioritize and organize work activities; also requires the ability to work effectively under pressure.
