Job Summary

The Case Manager will work as part of the Integrated Mobile Health Team to provide assertive mobile outreach to chronically homeless individuals in the Venice community and surrounding areas. The goal is to engage them in the service continuum and ensure their linkage to services that enable them to become housing ready. Once housing ready, the Case Manager will assist clients in applying for subsidized housing and linking them to available housing resources. The Case Manager will also provide ongoing intensive services to assist clients in retaining their housing and being good tenants and neighbors.

Key Responsibilities
  • Develop close working relationships with outreach workers, case managers, police, paramedics, and collaborating agencies.
  • Locate and focus efforts on specific chronically homeless persons who are considered most vulnerable.
  • Present findings to other collaborative agencies to develop a service plan of action that will expedite housing opportunities and housing stability.
  • Assist with developing and implementing client treatment plans that emphasize coordinated strategies for addressing mental health/substance abuse issues, relapse prevention, symptom/behavior reduction, medication management, and other risk factors that could impede permanent housing.
Requirements
  • Experience providing services to chronically homeless individuals or equivalent experience is desired.
  • Experience working with individuals with severe and persistent mental illness and/or co-occurring disorders is highly desirable.
  • Bachelor's degree from an accredited college or university in social work, human services, or a related field.
