Job Title: MDS Coordinator

We are seeking a highly skilled and compassionate MDS Coordinator to join our team at Summit LTC. As a key member of our nursing staff, you will be responsible for ensuring the highest quality of care for our residents.

Job Summary

The MDS Coordinator will be responsible for the completion of the Minimum Data Set (MDS) and the implementation and ongoing evaluation of the resident's comprehensive plan of care. This role requires strong clinical knowledge, excellent communication skills, and the ability to work effectively in a fast-paced environment.

Responsibilities
  • Complete the MDS and ensure accuracy and completeness of data
  • Develop and implement individualized care plans that meet the unique needs of each resident
  • Collaborate with the interdisciplinary team to ensure seamless care transitions and effective communication
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed
  • Ensure compliance with all applicable laws and regulations, including those related to resident rights and safety
Requirements
  • Current valid Texas driver's license or regular means of transportation to and from work
  • One year of experience as a nurse in a skilled rehabilitation, skilled nursing, or Long-Term Care environment
  • One year of clinical supervisory, management, or resource experience
  • Experience/expertise in gerontological nursing
  • Experience with electronic medical records
