We are seeking a highly skilled MDS Coordinator to join our team at Summit LTC NRH LLC. The successful candidate will be responsible for the completion of the federally required Minimum Data Set (MDS) while adhering to the Resident Assessment Instrument (RAI) and will be responsible for the implementation and ongoing evaluation of the resident's comprehensive plan of care.

Key Responsibilities:
  • Complete the federally required Minimum Data Set (MDS) in accordance with the Resident Assessment Instrument (RAI)
  • Implement and evaluate the resident's comprehensive plan of care
  • Ensure ongoing regulatory compliance with all applicable laws and professional standards of practice
  • Assume nursing leadership functions within the nursing team
  • Foster effective communication of resident needs and issues to the team
Requirements:
  • High school diploma or relevant qualification
  • Nursing Degree from an accredited college or university, or be a graduate of an approved RN or LVN program
  • Current valid Texas driver's license or regular means of transportation to and from work
  • One (1) year of experience as a nurse in a skilled rehabilitation, skilled nursing or Long-Term Care (LTC) environment
  • One (1) year of clinical supervisory, management, or resource experience
  • Experience/expertise in gerontological nursing
  • Experience with electronic medical records
