JOB SUMMARY:
This role is essential for overseeing, documenting, updating, and tracking all Minimum Data Set (MDS) paperwork for Medicare patients in accordance with state regulations and relevant authorities.
ESSENTIAL JOB FUNCTIONS:
To succeed in this position, an individual must effectively perform each critical function, with or without reasonable accommodation.
• Oversee and maintain the MDS/PPS program and its completion timeline.
• Create and coordinate suitable nursing care plans based on MDS/RUG categories.
• Independently analyze assessment data for complex patient issues and identify early subtle changes that influence RUG categories and patient care.
• Assess the effectiveness of nursing care plans; revise MDS/RAI processes within completion guidelines.
• Respond promptly to all emergency calls with appropriate interventions.
• Provide patient care, including administering treatments, medications, and offering social and emotional support.
• Perform additional duties as assigned.
JOB REQUIREMENTS:
• Successful completion of required training.
• Ability to manage multiple priorities effectively.
• Independent decision-making and discretion.
• Reliable transportation.
Required Computer Software/Equipment:

REQUIRED QUALIFICATIONS:
• License/Certification/Education:
o Completion of a Registered Nursing Program.
