Job Summary

Cambridge HS is seeking a highly skilled MDS Coordinator to join our team. As a key member of our healthcare team, the successful candidate will be responsible for coordinating the development and completion of resident assessments in accordance with federal, state, and local standards.

Key Responsibilities
  • Coordinate the RAI process, including the completion of MDS, CAA's, and the development of comprehensive care plans for each resident, following RAI guidelines and facility policies.
  • Ensure that resident care plans are being followed by the interdisciplinary team and monitor their progress to ensure compliance with the MDS process.
  • Complete medical forms, charts, and reports in an accurate and timely manner.
  • Assist the Director of Nursing (DON) or Assistant Director of Nursing (ADON) or supervisors as needed.
  • Understand long-term care reimbursement processes, including PDPM, Managed Care, and Medicaid.
  • Participate in scheduling resident care conferences.
  • Participate in the Quality Assurance and Performance Improvement (QAPI) process.
  • Monitor Quality Care Indicators.
  • Carry out quality improvement initiatives.
  • Abide by all facility policies and procedures, including maintaining confidentiality and not disclosing user ID codes and passwords.
  • Report any occupational exposures to blood, body fluids, or other hazardous materials to a supervisor immediately.
  • Participate in facility surveys (inspections) when required and assist with the plan of corrections and follow-up.
  • Attend meetings and serve on committees as requested.
Requirements
  • RN Nursing Degree or LVN from an accredited school or college.
  • Valid RN or LVN license in good standing.
  • Valid CPR and BLS card.
  • Minimum of 1 year of skilled nursing experience preferred.
  • Ability to speak, read, write, and comprehend the English language.
  • Ability to use a computer/tablet to enter resident data.
  • Proficiency with PCC a plus.
