Clinical Social Worker Job Description

We are seeking a skilled Clinical Social Worker to join our team at UCHealth Longs Peak Hospital. As a Clinical Social Worker, you will play a vital role in assessing patients and their families, caregivers, and legal representatives to arrange for necessary interventions.

Key Responsibilities:

  • Participate in case reviews to evaluate care management and progress toward goals.
  • Consult with healthcare team members to promote, monitor, and evaluate compliance with patient treatment plans.
  • Assist with discharge planning and processes, identifying appropriate resources and providing referrals as part of the discharge plan.
  • Administer and interpret tests and measures of psychosocial functioning to diagnose mental, emotional, behavioral, addictive, and developmental disorders and disabilities.
  • Supervise and train new staff, lower-level social workers, students, or interns.

Requirements:

  • Master's degree in Social Work.
  • State licensure as a Licensed Clinical Social Worker (LCSW).
