We have an exciting opportunity to join a well established hospital as a Hospital Social Worker The Hospital Social Worker will join a team of tenured medical professionals at an outstanding 25+ bed critical access hospital. The Hospital Social Worker plays a vital role in providing comprehensive psychosocial support and therapeutic interventions to patients and their families within the healthcare setting. Reporting to the Director of Social Services or Clinical Manager, this position involves conducting psychosocial assessments, developing individualized treatment plans, and providing counseling and support services to patients facing emotional, behavioral, or situational challenges.
The Hospital Social Worker collaborates closely with interdisciplinary healthcare teams to address the complex needs of patients, including coordinating care transitions, advocating for patient rights, and facilitating access to community resources and support services. Responsibilities also include crisis intervention, mental health assessment, and participation in interdisciplinary treatment planning meetings. Strong communication, assessment, and counseling skills, as well as a deep understanding of psychosocial dynamics and mental health issues, are essential for success in this role.
