Job Summary

Key Responsibilities
  • Collaborate with healthcare teams to identify patient needs and develop comprehensive care plans.
  • Conduct thorough psychosocial assessments and provide guidance on end-of-life care.
  • Facilitate communication between patients, families, and healthcare providers.
  • Develop and maintain information on community resources and referral requirements.
  • Document activities in medical records and maintain statistical data.
  • Participate in multidisciplinary care coordination rounds and quality improvement teams.
  • Provide crisis intervention and management services as needed.
Requirements
  • Master's degree in social work from a Council on Social Work Education-accredited program.
  • Two plus years of experience working as a Social Worker, preferably in a hospital setting.
  • LGSW, LCSW-C, or MSW required.
