About the Role

Point32Health, Inc. is seeking a highly skilled Nursing Care Manager to join our team. As a Nursing Care Manager, you will play a critical role in ensuring that our members receive timely and effective care management across the continuum.

Key Responsibilities
  • Assessment and Planning: Conduct thorough assessments of members' needs, develop and implement personalized care plans, and coordinate with healthcare providers to ensure seamless care transitions.
  • Care Coordination: Facilitate communication between members, caregivers, and healthcare providers to ensure that members receive the necessary care and support.
  • Health Education and Support: Provide members with education and support to help them manage their health conditions, adhere to treatment plans, and make informed decisions about their care.
  • Case Management: Identify and address barriers to care, develop strategies to prevent hospital readmissions, and ensure that members receive the necessary resources and support to achieve optimal health outcomes.
  • Collaboration and Communication: Work closely with interdisciplinary teams, including physicians, nurses, social workers, and other healthcare professionals, to ensure that members receive comprehensive and coordinated care.
Requirements
  • Education: Bachelor's degree in Nursing or related field.
  • Experience: Minimum 5 years of experience in nursing, with a focus on case management, care coordination, or a related field.
  • Certifications: Current RN licensure and national certification in case management (CM) or a related field.
  • Skills: Strong clinical knowledge, critical thinking skills, and ability to communicate effectively with members, caregivers, and healthcare providers.
