Key Responsibilities:

  • Conduct thorough assessments and evaluations to identify patient needs
  • Develop and implement individualized care plans to meet patient needs
  • Monitor patient progress and adjust care plans as needed

Requirements:

  • Active PN License: Current and active license to practice as a LPN / LVN in the state of Massachusetts
  • CPR/BLS Certification: Current certification in CPR/BLS
  • Experience: Previous experience working in long-term care settings is preferred
