Job Title: Child and Adolescent Mental Health Case Manager

StarCare Specialty Health System is seeking a qualified and compassionate Child and Adolescent Case Manager to provide supportive services to children and their families.

About the Role:

The C&A Case Manager provides services to children in the community and in clinical settings that are related to an approved individual plan of service. The role involves providing hands-on, in-vivo skills training, symptom management, support, medication monitoring, crisis intervention, and peer support.

Responsibilities:
  • Provide routine and intensive case management services, including evaluation, monitoring, linking, and coordinating consumers on assigned caseload.
  • Ensure clients are aware of resources necessary to reach personal outcomes and communicate pertinent clinical information.
  • Provide face-to-face contact with clients and their support systems.
  • Refer to inpatient services as needed.
Requirements:
  • Graduation from an accredited four (4) year college or university with major course work in a related field.
  • One (1) year full-time experience in a related field.
  • Knowledge and skill in operating standard office equipment and personal computer with Microsoft Office products.
  • Knowledge of human service delivery systems.
  • Ability to communicate effectively, both orally and in writing.
  • Ability to organize, prioritize a variety of assignments, and manage time effectively.
  • Ability to maintain an effective working relationship with clients, other employees, and the public.
  • Must have a current Texas driver's license and be insurable under Center insurance.
  • Must pass a pre-hire drug screen and criminal background check.
