As a Clinical Care Coordinator, you will play a vital role within an Integrated Care Model team, addressing both medical requirements and social determinants of health. Your responsibilities will include communicating essential information regarding members' needs, advocating for optimal care solutions, and ensuring access to appropriate services tailored to their requirements.

This role is primarily field-based, with a home office setup.

Key Responsibilities:

  • Evaluate, plan, and execute personalized care strategies for members, focusing on the most suitable and least restrictive level of care.
  • Oversee the care plan throughout the entire continuum, serving as a single point of contact.
  • Facilitate communication among all stakeholders to ensure timely delivery of quality coordinated care and services to all members.
  • Advocate for patients and their families to ensure their needs and preferences are fully represented and supported by the health care team.
  • Anticipate spending approximately 50-80% of your time in the field, visiting members in their residences or long-term care facilities.

Required Qualifications:

  • Current, unrestricted RN license in MA with a minimum of 2 years of clinical experience.
  • At least 2 years of experience in long-term care, home health, hospice, community health, or assisted living settings.
  • Proficiency in MS Word (creating and editing documents) and Outlook (email and calendar management) for at least 2 years.
  • Willingness to travel within the assigned region to visit members in various settings, including their homes and community centers.
  • Access to high-speed internet at home.

Preferred Qualifications:

  • Bachelor of Science in Nursing (BSN) or actively pursuing a BSN.
  • Experience with electronic health records.
  • Previous field-based work experience.
  • Background in managing populations with complex medical or behavioral health needs.
  • Bilingual abilities.
