Key Responsibilities- Facilitate care coordination and transition planning to promote timely throughput, safe discharge, and prevent avoidable readmissions.
- Conduct accurate medical necessity screening and submission for Physician Advisor review.
- Collaborate with physicians, office staff, and ancillary departments to ensure seamless care coordination.
- Lead and facilitate multi-disciplinary patient care conferences to optimize patient outcomes.
- Develop and implement effective transition plans to ensure patient safety and satisfaction.
- Identify and refer complex patients to Social Work Services as needed.
- Communicate effectively with patients, families, and caregivers to ensure understanding and engagement in the care plan.
- Document all patient interactions and care coordination activities in a timely and accurate manner.
- Graduate of an accredited school of nursing.
- 2 years of acute hospital patient care experience.
- Active RN license for the state of Massachusetts.
- Preferred: Academic degree in nursing (bachelor's or master's) and/or accredited Case Manager (ACM) certification.
