  • Lead and facilitate multi-disciplinary patient care conferences to optimize patient outcomes.
  • Develop and implement effective transition plans to ensure patient safety and satisfaction.
  • Identify and refer complex patients to Social Work Services as needed.
  • Communicate effectively with patients, families, and caregivers to ensure understanding and engagement in the care plan.
  • Document all patient interactions and care coordination activities in a timely and accurate manner.
Requirements
  • Graduate of an accredited school of nursing.
  • 2 years of acute hospital patient care experience.
  • Active RN license for the state of Massachusetts.
  • Preferred: Academic degree in nursing (bachelor's or master's) and/or accredited Case Manager (ACM) certification.
