This is a per diem/on call position supporting our Beaverton site and surrounding areas as needed. The RN provides care management, care coordination, skilled care and services for at-risk frail elder patients with complex medical and psychosocial issues.
Key Responsibilities- Provide care management and care coordination for patients with complex medical and psychosocial issues.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with the interdisciplinary team to ensure comprehensive care and services are provided to patients.
- Assist providers in identifying needs and coordinating access to appropriate care and services.
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
- Oregon Registered Nurse License
- National Provider BLS - American Heart Association within 30 days of hire
- Relevant certifications applicable to the position, as specified by the Nurse Manager
- Bachelor's Degree in Nursing
- 1 year Home health RN experience
