Monogram Health is seeking dedicated Nurse Practitioners and Physician Assistants who are passionate about enhancing the lives of patients.

The Advanced Practitioner at Monogram Health plays a crucial role within an integrated Care Team, collaborating closely with a Registered Nurse and a Social Worker.

Our focus is on patients facing multiple chronic conditions, and our Nurse Practitioners and Physician Assistants are instrumental in improving their quality of life at home while managing the progression of kidney disease.

Your Contribution


In this role, you will provide personalized, compassionate medical care primarily to individuals with Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD).

Your responsibilities will include patient care, maintaining precise and up-to-date patient records, scheduling, and conducting follow-up appointments as necessary.

Your expertise as a healthcare professional is urgently needed.

In many healthcare systems, the patient often becomes secondary due to processes and incentives that do not foster long-term positive outcomes.

Roles and Responsibilities

  • Conduct comprehensive assessments, including annual wellness exams, both in-home and virtually
  • Educate and counsel patients and families about available resources and programs to support healthier living
  • Document key information such as chief complaints, diagnoses, medical history, and treatment plans
  • Coordinate care with primary care providers, specialists, and ancillary services
  • Ensure timely completion of all documentation and paperwork
  • Uphold quality of care standards as established by the practice
  • This position is remote within the state of employment, with occasional training and meetings outside the state
  • Deliver evidence-based care that minimizes avoidable hospitalizations and prioritizes patient health and satisfaction
  • Prescribe medications, order diagnostic tests, and collaborate with the Monogram physician
  • Demonstrate effectiveness through improved patient outcomes, measured and reported regularly
  • Close care gaps by educating patients on preventive measures and coordinating diagnostic testing
  • Assist patients in accessing educational resources
  • Participate in integrated care team meetings
  • Maintain knowledge of disease diagnosis and prevention
  • Assess patient health status and develop appropriate treatment plans
  • Implement care plans in alignment with best practices
  • Follow up and evaluate patient progress
  • Perform additional duties as assigned
Position Requirements

  • Bilingual (English/Spanish) preferred
  • Active and unrestricted Nurse Practitioner or Physician Assistant license
  • Board certification relevant to licensure (NP: ANCC/AANP; PA: NCCPA)
  • Current and unrestricted DEA certificate
  • Ability to work independently and practice autonomously
  • Valid driver's license, car insurance, and access to transportation
  • Proficiency with medical instruments and required equipment
  • Familiarity with data management systems and medical records technology
  • Strong communication skills, both verbal and written, with various stakeholders
  • Understanding of federal and state regulations, including HIPAA and OSHA
  • Minimum of 2 years of direct patient care experience
  • Experience in Managed Care/IPA/Health Plan settings
  • Experience conducting comprehensive wellness visits, either virtually or in-home
