Clinical Liaison and Care Coordinator

As a Registered Nurse Case Manager at Northwell Health, you will serve as a liaison between patients and healthcare facilities/physicians, ensuring a continuum of quality patient care throughout hospitalization. Your responsibilities will include overseeing provisions for patient discharge, assessing and evaluating the appropriateness of care, and collaborating with healthcare teams to ensure seamless transitions.
Key Responsibilities
- Coordinate patient care and discharge planning to ensure timely and effective transitions
- Collaborate with healthcare teams to assess and evaluate patient needs and develop individualized care plans
- Communicate effectively with patients, families, and healthcare providers to ensure clear understanding of care plans and progress
- Identify and address potential barriers to care, including social determinants and healthcare access issues
Requirements
- Current RN licensure in the state of New York
- Minimum 2 years of experience in case management or a related field
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities
