Job Title: Per Diem Pharmacist

Penn State Health is seeking a highly skilled and dedicated Per Diem Pharmacist to join our team at St. Joseph Medical Center in Reading, Pennsylvania.

Job Summary:

The Per Diem Pharmacist will be responsible for providing exceptional patient care and medication management services to our patients. This is a per diem position, requiring flexibility to work varied shifts, including days, evenings, and weekends.

Responsibilities:
  • Provide accurate and timely medication dispensing and counseling services to patients.
  • Conduct thorough medication reviews and assessments to ensure patient safety and optimal medication therapy.
  • Collaborate with healthcare teams to develop and implement individualized patient care plans.
  • Participate in quality improvement initiatives to enhance patient outcomes and pharmacy services.
  • Maintain accurate and up-to-date patient records and medication profiles.
Requirements:
  • PharmD degree or Bachelor's degree in Pharmacy required.
  • Current Pennsylvania licensure or eligibility required.
  • Previous hospital pharmacy experience preferred.
  • Basic Life Support (BLS) certification preferred.
