We are seeking a highly skilled Medication Therapy Expert to join our team at Duke University. As a Clinical Pharmacy Professional, you will play a critical role in providing comprehensive medication therapy management services to patients across the region.

About the Role

This is an exciting opportunity to work with a talented multidisciplinary team to improve patient outcomes through evidence-based pharmaceutical care practices. You will be responsible for reviewing and monitoring patient medication regimens, advising healthcare teams on optimal medication use, and ensuring compliance with regulatory requirements.

Key Responsibilities

- Review and monitor patient medication therapy orders written by physicians and other caregivers
- Provide medication information and education to patients, families, and healthcare professionals
- Conduct medication histories and assess compliance to achieve desired outcomes
- Participate in patient care rounds with multidisciplinary teams to evaluate patient progress
- Collaborate with physicians and nursing staff to develop individualized medication plans

Requirements

To be successful in this role, you must have a Doctor of Pharmacy degree or BS Pharmacy degree with relevant experience. You should be licensed as a pharmacist or eligible to practice pharmacy in North Carolina. A strong understanding of pharmacokinetics, disease states, and drug interactions is essential. Excellent communication and interpersonal skills are also required.

Duke University offers a competitive salary range of $ $120,000 - $150,000 per year, depending on qualifications and experience.
