Field RN Home Health Visits Specialist

We are seeking a skilled and compassionate Field RN Home Health Visits Specialist to join our team at Activa Home Health. As a Field RN Home Health Visits Specialist, you will be responsible for providing high-quality care to patients in their homes, utilizing individualized Plans of Care to ensure patient safety, well-being, and satisfaction.
Key Responsibilities- Provide Safe and Effective Care: Develop and implement individualized Plans of Care to meet the unique needs of each patient, ensuring safe and effective care in the home setting.
- Communicate Effectively: Maintain open and prompt communication with patients, families, and healthcare teams to ensure seamless care coordination and effective management of cases.
- Manage Patient Care: Provide skilled nursing care, including medication administration, wound care, infusion therapy, and other interventions as needed, in accordance with the nursing scope of practice and individualized Plan of Care.
- Collaborate with Healthcare Teams: Work collaboratively with Nurse Supervisors, Managers of Clinical Practice, and other healthcare professionals to ensure comprehensive care and effective case management.
- Demonstrate Empathy and Caring: Provide patient-centered care, demonstrating empathy, caring, and self-confidence, with a focus on delivering exceptional patient outcomes.
- Current RN License: Possess a current and active RN license in the state of Florida.
- Wound Care and Infusion Skills: Possess wound care and infusion skills, with the ability to provide safe and effective care in these areas.
- Excellent Communication Skills: Demonstrate excellent communication skills, with the ability to work effectively with patients, families, and healthcare teams.
- Home Health Experience: Preferably 1 year of experience in home health, with a strong understanding of the unique needs and challenges of patients in this setting.
