Job Summary:

We are seeking a certified Geriatric Care Manager dedicated to delivering the highest quality in-home care management. This position has flexible hours, including day, evening, weekend, or a combination.

Key Responsibilities:

  • Direct care to clients and their families using a multidisciplinary approach.
  • Conduct start-of-care admissions and assessments to identify problems and necessary services.
  • Develop a professional care plan from assessment findings and in collaboration with the client's physician.
  • Ensure in-home care delivery is in accordance with each client's contractual program.
  • Communicate explanations of the care plan and all subsequent costs of services to clients and/or families, attorneys, trust officers, or guardians.

Requirements:

  • Certified Geriatric Care Manager (CGCM)
  • Registered Nurse with 1 year of experience in home case/care management
  • Geriatric Care Manager Certification
  • Experience working with the adult population
  • Knowledge of community health, social service agencies, and additional community resources
  • Ability to travel to clients' residences within 30 to 40 miles
  • Excellent communication and interpersonal skills with the ability to quickly build rapport
  • Ability to work with minimal supervision within the role and scope
  • Ability to use a variety of electronic information applications/software programs, including electronic medical records
  • Valid driver's license, car insurance, and access to an automobile
  • Licensed to work in the State of Florida - will need ACHA Level II Background clearance

Preferred Qualifications:

  • BSN
  • Geriatric Care Manager Certification
  • Bilingual - English, Spanish
  • Experience with Private Pay Home Healthcare
