Key Responsibilities- Leadership and Management
- Provide oversight and guidance to a team of healthcare professionals, including RNs, LPNs, and CNAs.
- Develop and implement effective strategies to improve patient outcomes and satisfaction.
- Collaborate with the interdisciplinary healthcare team to ensure seamless care coordination and planning.
- Patient Care
- Assess patient needs and develop individualized plans of care in collaboration with the healthcare team.
- Ensure that patient care services are delivered in accordance with regulatory requirements and Interim HealthCare's policies and procedures.
- Monitor patient progress and make adjustments to care plans as needed.
- Quality Assurance and Performance Improvement
- Participate in the Quality Assessment and Performance Improvement (QAPI) program to identify areas for improvement and implement changes to enhance patient care and outcomes.
- Conduct regular audits and reviews to ensure compliance with regulatory requirements and Interim HealthCare's policies and procedures.
- Education and Training
- Develop and implement education programs for patients, families, and healthcare professionals to promote patient education and empowerment.
- Provide ongoing training and support to healthcare professionals to ensure they have the necessary skills and knowledge to deliver high-quality patient care.
- Education and Licensure
- Graduate of an accredited nursing program and active RN license in Florida.
- Experience
- Minimum of 3 years of experience in home healthcare, with a minimum of 1 year of supervisor experience.
- Certifications and Skills
- CPR certification.
- Knowledge of state and federal home health regulations.
- Strong leadership and management skills, with the ability to motivate and inspire a team.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
