Job Summary

We are seeking an experienced Occupational Therapist to join our team at Liberty Health. As a Home Care Occupational Therapist, you will play a vital role in providing patient care services and direct treatment to our patients in the comfort of their own homes.

Key Responsibilities
  • Provide patient care services and direct treatment to patients in their homes
  • Evaluate patients' new to service and develop a plan of care based on their diagnosis and assessment findings
  • Establish measurable goals and document patient care data accurately and thoroughly
  • Coordinate with Certified Occupational Therapy Assistants (COTAs) and provide monthly supervision
  • Communicate with intercompany and intracompany contacts as necessary
  • Discuss company and third-party reimbursement policies and guidelines with patients and families
  • Coordinate referrals to appropriate disciplines and provide in-service education and public education
Requirements
  • Minimum bachelor's degree in occupational therapy from an accredited school of occupational therapy
  • Successful completion of the National Certification Exam and current licensure from the state where therapy will be provided
  • Prefer a minimum of one year's therapeutic experience in a non-home health care environment
  • Strong knowledge of theory and concepts relating to the practice of occupational therapy
  • CPR certification and strong communication and organization skills
  • Minimum of twelve hours of continuing education annually
