Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Hospice Nurse. As a member of our team, you will play a vital role in providing high-quality care to patients and their families in the comfort of their own homes.

Responsibilities
  • Assist in the planning, implementation, and coordination of patient care services
  • Provide emotional, spiritual, and physical support to patients and their families
  • Collaborate with interdisciplinary teams to ensure seamless care transitions
  • Document patient progress and outcomes
Requirements
  • Current New York State Registered Nurse License/Permit
  • Two years of experience in nursing, including medical-surgical, oncological, palliative, and community home health/hospice
  • At least one year of community hospice or home health experience preferred
  • Hospice experience preferred
  • NYS driver's license preferred
