Job Title: Hospice Nurse Practitioner

Swan Hospice is seeking a skilled and compassionate Hospice Nurse Practitioner to join our team.

Job Summary:

The Hospice Nurse Practitioner will be responsible for providing high-quality medical care to patients with terminal illnesses, working closely with the Hospice Medical Director and interdisciplinary team to ensure seamless care coordination.

Key Responsibilities:
  • Re-certification of Patients for Hospice Care:
    • Reviews patient's medical eligibility for hospice services by performing a nursing examination and documenting the same in the clinical record.
    • Performs and documents each face-to-face encounter and each subsequent recertification encounter no more than thirty (30) calendar days prior to the start of each benefit period.
  • Intervention/Patient Care Coordination:
    • Performs medical examinations and patient home visits beyond those required for the face-to-face encounters.
    • Assists in managing the patient's terminal condition, including prescribing medication or care by coordinating with the attending physician or Hospice Medical Director.
    • Provides consultation for palliative and end-of-life care issues on an as-needed basis and as directed by the Hospice Medical Director.
  • Leadership/Professionalism:
    • Maintains current knowledge of hospice principles and practices.
    • Develops and maintains cooperative working relationships with hospice staff.
    • Able to exercise independent judgment, critically analyze situations, and make recommendations using appropriate problem-solving skills.
    • Able to function effectively as a member of a team.
    • Must have excellent interpersonal skills and the ability to work and communicate within all levels of the organization.
    • Effectively communicates in interpersonal, professional, and public relationships.
    • Participates in the Quality Assessment/Performance Improvement Program of the agency as appropriate and needed.
Requirements:

To be successful in this role, you will need:

  • A Master's degree in Nursing or a related field.
  • Current licensure as a Nurse Practitioner in the state of practice.
  • Experience working in hospice care or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team environment.
