LPN - Licensed Practical Nurse - Forest City - FT Evenings

The Licensed Practical Nurse (LPN) plays a vital role in providing high-quality care to residents of all ages in our long-term care facility. As a member of our interdisciplinary care team, you will work closely with healthcare professionals to ensure the health, comfort, and safety of our residents.
Responsibilities- Provide professional nursing care to residents, including administering medications, treatments, and performing testing to evaluate resident status.
- Contribute to the assessment of residents and develop individualized care plans to meet their unique needs.
- Counsel residents, family members, and/or resident advocates on prevention and treatment plans, under the direction of a registered nurse, advanced practice provider, or physician.
- Perform other related duties as assigned, including documenting resident interactions and outcomes, and maintaining accurate records.
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
- Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges.
- Obtains and subsequently maintains required department-specific competencies and certifications.
