LPN - Licensed Practical Nurse - Forest City - FT Evenings

The Licensed Practical Nurse (LPN) plays a vital role in providing high-quality care to residents of all ages in our long-term care facility. As a member of our interdisciplinary care team, you will work closely with healthcare professionals to deliver compassionate and individualized care to our residents.
Responsibilities- Provide professional nursing care to residents, including administering medications, treatments, and performing various medical procedures.
- Conduct thorough assessments of residents to identify their physical and psychosocial needs, and develop individualized care plans to address these needs.
- Collaborate with the care team to ensure seamless transitions of care and effective communication with residents, families, and healthcare providers.
- Document resident interactions, outcomes, and care provided, maintaining accurate and up-to-date records.
- Participate in ongoing education and training to enhance your skills and knowledge, staying current with best practices and industry developments.
- Graduate of an accredited practical nursing program with a current LPN license.
- Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with residents, families, and colleagues.
- Ability to maintain confidentiality and handle sensitive information with discretion.
