Position Overview

The Clinical Transition Specialist is a seasoned sales professional who employs effective sales strategies to promote Option Care products and services to patients preparing for discharge. This role involves educating patients, their families, and healthcare facility staff on how our services and products will be implemented in alternative care settings. The Clinical Transition Specialist plays a crucial role in ensuring the appropriate placement of patients within the Home Health Care framework by conducting assessments, gathering pre-admission data, and collaborating with both internal and external partners to guarantee quality service and a successful treatment plan.

Key Responsibilities

  • Actively nurture and expand relationships with referral sources to drive sales and patient admissions, serving as the primary contact for education and support.
  • Engage with local service providers daily to advocate for Option Care services that can facilitate the care of discharging patients, conducting assessments to determine patient eligibility for alternative site care.
  • Utilize sales techniques to inform patients, their families, and facility staff about the comprehensive services and products offered by Option Care.
  • Deliver in-person education to patients and caregivers, aiming for timely discharges and fostering therapy independence.
  • Maintain effective communication with agency personnel, medical teams, patients, and families throughout the discharge process to implement a successful treatment plan.
  • Collect, review, and finalize pre-admission information while ensuring necessary approvals are secured.
  • Collaborate with the Account Manager and Regional Sales Director to develop and execute strategic business growth plans.
  • Maintain regular communication with the Account Manager to identify opportunities, assess progress, and provide insights on promoting Option Care services.
  • Work alongside the Account Manager and marketing team to deliver educational and promotional initiatives to patients, caregivers, and referral sources.
  • Review patient medical records to gather relevant medical history and insurance information, communicating this to the patient registration department.
  • Proactively coordinate care transitions with referral sources and internal teams to ensure seamless patient transfers to home or alternate care settings.
  • Safeguard patient confidentiality and adhere to legal guidelines for protecting sensitive information.
  • Provide guidance and feedback to healthcare providers regarding patient assessments and the transition process to home care.
  • Act as a liaison for coordination and communication with other healthcare providers.
  • Facilitate arrangements for any necessary medical supplies or equipment.

Supervisory Role

This position does not include supervisory responsibilities.

Essential Qualifications

  • Licensed Registered Nurse (RN) or licensed Pharmacist in the state of practice with a minimum of 2 years of experience in the healthcare sector.
  • Proven ability to establish and maintain relationships with individuals at all organizational levels and within the business community.
  • Knowledge of standard practices relevant to all services offered, as well as current applicable standards.
  • Experience in delivering customer service to both internal and external clients, ensuring quality service and evaluating customer satisfaction.
  • Basic proficiency in PC skills, including internet usage, email, and Microsoft Office applications.
  • Ability to identify operational challenges and recommend strategies for process improvement.
  • Access to reliable transportation for travel to care facilities and hospitals, including a valid driver's license and proof of insurance if using a personal vehicle.
  • Strong organizational and presentation skills.

Travel Requirements

This role involves travel to partner facilities, community hospitals, and medical offices to promote Option Care Services, process referrals, and provide educational support to patients and caregivers.

Preferred Qualifications

  • Experience in home or alternate site infusion or discharge planning.
  • Proven track record in expanding service provider partnerships.
Compensation and Benefits

Salary will be determined based on the applicant's education, experience, knowledge, skills, and abilities, as well as internal equity and market alignment. Minimum pay starts at $66,877.92.
