Job Summary

You will work closely with our care management team, providers, and community resources to ensure our members receive the best possible care.

Key Responsibilities:

  • Coordinate care management services for members with complex medical and behavioral conditions
  • Work closely with care management team, providers, and community resources to ensure members receive best possible care
  • Develop and implement care plans in conjunction with members, providers, and community resources
  • Monitor and evaluate member outcomes and make adjustments to care plans as needed
  • Represent Community Health Plan of Washington at regional meetings and conferences

Requirements:

  • Bachelor's degree in nursing, social work, or related field
  • Current, unrestricted license in Washington state as RN, LSWAA, LSWAIC, LMHC, LMHP, or LMFT
  • Minimum 1 year of case management experience
  • Minimum 2 years of experience in care management workflow systems, NCQA standards, and presenting to organizations

Preferred Qualifications:

  • Case management certification
  • Bilingual
  • Experience in managed care (Medicaid/HCA)
  • Experience with Care Management software applications
