We are seeking a highly skilled and compassionate Intensive Case Manager to join our team at Community Health Action of Staten Island. The successful candidate will play a crucial role in supporting clients in our Alternative to Incarceration (ATI) program, focusing on those at a high risk of recidivism.
Key Responsibilities- Conduct thorough assessments and develop individualized service plans to ensure clients receive comprehensive care.
- Facilitate access to substance use treatment, mental health services, and other recovery supportive services.
- Collaborate with court personnel, defense attorneys, and judges to ensure seamless program enrollment and progress tracking.
- Provide trauma-informed supportive services, including individual and group sessions, to promote client well-being and stability.
- Document client progress and present reports to stakeholders, including the court.
- Participate in quality assurance activities and contribute to quality improvement projects.
- Master's degree in social work, mental health counseling, or a related field.
- At least 2 years of experience providing counseling services to individuals with substance abuse or mental health issues.
- Experience utilizing trauma-informed care and harm reduction models.
- Bilingual (English/Spanish) highly preferred.
- Strong assessment skills, care planning, and computer literacy.
- Ability to work independently and as part of a team, with a strong commitment to community health and wellness.
