Job Title: Case Planner Job Summary: At MercyFirst, we are seeking a dedicated and compassionate Case Planner to join our team. As a Case Planner, you will work with children and youth who reside in foster homes, providing intensive support to ensure their well-being and progress towards permanency. Responsibilities: * Provide regular casework sessions with clients, conducting home visits and maintaining contact with birth parents and foster parents * Develop and implement individualized plans to address the unique needs of each client * Collaborate with Socio-Therapists to provide comprehensive support to clients and their families * Document all contacts and case activities in the State system of Connections and Agency patient tracking systems * Prepare and submit required documentation, including progress notes, psychosocial reports, and permanency hearing notes Requirements: * Bachelor's degree in social work or related field * 1 year of experience in serving children and/or families * Knowledge of child development stages and family dynamics * Excellent communication and documentation skills * Proficiency in Microsoft Word and Outlook Benefits: * Comprehensive health insurance package * 403B retirement benefits * Employer-paid life insurance and long-term disability insurance * Generous paid time off and 12 paid holidays * Free employee assistance program Salary Range: $54,000 - $55,000 per year Hiring Incentive: $1,000 after 500 worked hours About MercyFirst: MercyFirst is a not-for-profit human and social service agency that has been serving children and families in need since 1894.
