Patient Monitoring Specialist

Position Description:
Circadia Health is revolutionizing remote patient monitoring for post-acute care and skilled nursing facilities.
We're seeking a highly motivated and detail-oriented Remote Patient Monitoring Coordinator who is passionate about patient care and eager to gain valuable experience in improving patient outcomes.
As a key member of our interdisciplinary team, you will lead initial patient visits, ensure their successful completion, and collaborate closely with our team to deliver the highest standards of care.
Key Responsibilities:
Travel to SNFs:
Conduct initial remote patient monitoring visits at various skilled nursing facilities within a designated region.
Patient Assessments:
Take vital signs and document findings accurately, ensuring the highest level of patient care.
Virtual Visit Set Up:
Assist with virtual patient visits, ensuring all necessary technology is functioning properly and patients are comfortable with the process.
Technical Support:
Troubleshoot technical issues that may arise during virtual visits and assist patients in resolving them efficiently.
Equipment Management:
Oversee company equipment, ensuring all items are in working condition and coordinate any necessary repairs.
Excellent Communication Skills:
Effectively interact with patients, conveying information to enhance their understanding and experience.
Qualifications:
Certification as a Certified Nursing Assistant (CNA) or Medical Assistant (MA) or willingness to obtain
Valid driver's license and willingness to drive to various skilled nursing facilities
Ability to work independently with minimal supervision
Comfort using technology such as Google Suite, multiple EMRs, and Slack
English required, Spanish preferred
$20 - $30 an hour
