Job Summary:

We are seeking a skilled and compassionate Certified Nursing Assistant to join our team at JWCH Institute, Inc. as a Patient Care Specialist. In this role, you will provide high-quality patient care and support to individuals in our Recuperative Care Program.

Key Responsibilities:

  • Provide Personal Care: Assist patients with daily activities of living, including bathing, dressing, and grooming.
  • Monitor Patient Conditions: Observe and document patient response to care, reporting any changes or concerns to the LVN Supervisor.
  • Support Patient Navigation: Assist patients with navigating around the facility for meals, rehabilitative activities, and community appointments.
  • Assist with Meals and Hygiene: Assist patients with meals and hygiene activities, promoting patient safety and environmental cleanliness.
  • Document Patient Information: Accurately document patient information, following established policy and procedures.
  • Collaborate with the Care Team: Work collaboratively with the patient care team to support patient health and safety.

Requirements:

  • Education: Minimum High School diploma or equivalence.
  • Licensure: Licensed as a Certified Nurse Assistant in accordance with the laws of the State of California.
  • Experience: Minimum one-year experience in a healthcare setting.
  • Certifications: Current BLS Certificate from the American Heart Association.
  • Language Skills: Bilingual English/Spanish a plus.
