Responsibilities

The successful candidate will assist clients with referrals to health care providers and community resources based on their individual needs. This may involve working with justice-involved, homeless, or pregnant individuals who require specialized support.

You will also be responsible for fostering partnerships with local organizations and healthcare providers to ensure seamless care coordination. A strong understanding of substance use disorders and co-occurring conditions is essential for success in this role.

Compensation

We offer a competitive salary range of $45,000 - $55,000 per year, depending on experience. Please note that this is a full-time position, and we provide a comprehensive benefits package, including opportunities for professional growth and development.
