A Community of Friends is seeking a skilled Case Manager I to provide individualized support and services to tenants in our permanent supportive housing program. As a key member of our team, you will work closely with tenants to help them achieve their goals and improve their overall well-being.

Key Responsibilities:
  • Provide comprehensive case management services, including intake, assessment, goal setting, and monitoring
  • Develop and implement individualized service plans to meet the unique needs of each tenant
  • Collaborate with internal and external stakeholders to ensure seamless delivery of services
  • Document all tenant interactions and maintain accurate records in our Homeless Management Information System (HMIS)
  • Participate in regular team meetings and contribute to the development of program policies and procedures
Requirements:
  • Bachelor's degree in Social Services or related field
  • At least two years of experience in case management or a related field, preferably with individuals experiencing homelessness or mental illness
  • Strong knowledge of evidence-based practices and motivational interviewing techniques
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations
  • Valid California driver's license and access to a personal vehicle
Preferred Qualifications:
  • Bilingual (English/Spanish) or proficiency in another language
  • Experience working in a permanent supportive housing program or similar setting
  • Knowledge of the Coordinated Entry System and VI-SPDAT
  • Proficiency in Microsoft Office and HMIS
