Job Summary

The Case Manager serves as a vital member of the St. Joseph Center team, providing comprehensive support to homeless families experiencing domestic violence. This role involves delivering high-quality case management services, ensuring the delivery of quality care, and adhering to the mission of St. Joseph Center.

Key Responsibilities
  • Evaluate families for permanent housing in conjunction with the Program Manager and case management staff.
  • Manage a caseload of up to 25 families, establishing a case management and safety plan for each family.
  • Collaborate closely with the DV coordinator to provide referrals to necessary services, including health, mental health, education, employment, and vocational training.
  • Assist families with identifying appropriate housing and facilitating lease-up, including negotiating rental agreements with landlords.
  • Provide ongoing case management to families following placement.
Qualifications
  • Highly motivated and self-starting individual with excellent administrative and organizational skills.
  • Knowledge of case management principles and intervention techniques specific to homeless populations or under-served groups.
  • Knowledge of housing market and housing programs is desirable.
  • Ability to work flexible hours, including early mornings and evenings.
  • Bi-lingual Spanish/English is desirable.
Experience and Education
  • One to two years' experience providing case management services and advocacy with homeless and low-income families and/or families experiencing domestic violence.
  • Bachelor's degree from an accredited college or university in social work, human services, psychology, or a related field.
Computer Skills and Certifications
  • Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook) and widely supported internet browsers.
  • Valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed.
