Job Summary

We are seeking a highly skilled and compassionate Post-Transplant Coordinator to join our team at Houston Methodist. As a key member of our transplant team, you will play a vital role in ensuring the seamless coordination of care for our patients undergoing transplantation.

Key Responsibilities
  • Coordinate all aspects of the peri-operative and post-transplant phases of transplantation, including patient education, medication management, and follow-up care.
  • Collaborate with the transplant team to ensure effective communication and coordination of care.
  • Provide emotional support and education to patients and their families throughout the transplant process.
  • Monitor patient progress and identify potential complications, coordinating required care with the transplant team.
  • Maintain accurate and comprehensive documentation of patient information, including transplant history, medication regimens, and follow-up care.
Requirements
  • Bachelor's degree in Nursing or related field.
  • Four years of nursing experience, with at least two years in a transplant or ICU setting.
  • Current RN licensure in Texas or Compact State licensure within 60 days.
  • BLS certification (AHA).
