Job Summary

As a skilled Abdominal Transplant Coordinator RN, you will play a vital role in the care and management of transplant patients. Your primary responsibility will be to perform initial assessments, assist patients through the transplant process, and provide follow-up care post-transplant.

Key Responsibilities
  • Perform initial assessments of patients' biophysical and psychosocial status, ensuring appropriate actions are taken in relation to the nursing process.
  • Provide pre- and post-transplant interventions within the limits of licensure and/or scope of practice.
  • Collaborate with other healthcare providers to achieve outcomes and plan, coordinate, and organize discharge plans with inpatient nursing staff to ensure continuity of care.
  • Direct and provide guidance to staff, case managers, and physicians to ensure patient care delivery to established standards of practice.
  • Ensure safe care to patients, staff, and visitors, adhering to all Memorial Hermann policies, procedures, and standards within budgetary specifications.
Requirements
  • Two (2) years of clinical experience as a Critical Care RN.
  • Experience as a Transplant Coordinator is preferred.
  • BSN degree required.
  • Current State of Texas license to practice professional nursing.
